Intensive Care Coordination (ICC)

Intensive Care Coordination (ICC) is a state wide initiative to create more in-home services and community resources for children who are at risk of being placed or who are currently placed in a group facility.

The goal of the ICC is to successfully reunite and help transition children from residential facilities or group homes back to their families, as well as to work with their families to keep the children in their home and their community permanently. These goals are accomplished through a collaborative effort that assesses the family’s and child’s needs and working with them to build on their strengths.

All children receiving ICC services are also be apart of a Family Assessing and Planning Team (FAPT) which helps coordinate services, and identify what will be most helpful within the child’s home. Community Service Boards (CSB’s) across Virginia provide ICC, along with other private providers.
